A multidisciplinary team, including the parent/guardian/other education rights holder, develops the IFSP from an assessment of the unique needs of the infant or toddler. The IFSP also identifies the services appropriate to meet such needs.[1] The IFSP is evaluated at least once a year.[2] In addition, a review of the plan is provided to the family every 6 months, or more often when needed or if the parent/guardian/other education rights holder requests such a review.[3]
