The IFSP shall be in writing and shall include the following:
- a statement of the infant or toddler’s present levels of development;
- a statement of the family’s concerns, priorities and resources;
- a statement of the major outcomes to be expected, and the criteria, procedures and timelines used to evaluate these outcomes;
- a statement of specific services including how often, how much, and the method of delivery;
- a statement of the “natural environments” in which early intervention services shall appropriately be provided, including a justification of the extent, if any, to which the services will not be provided in a natural environment;
- dates for starting services and the anticipated length, duration, and frequency of the services;
- name of agency responsible for providing the identified services;
- name of the service coordinator; and,
- a transition plan to other appropriate services.[1]
Parents/Guardians/Other Education Rights Holders must sign the IFSP to give consent for services to begin.[2]
